The GLP-1 Plateau: Why the Scale Stops and What To Do
Somewhere between month six and month eighteen, most people on GLP-1 medication meet the flat stretch: same shots, same habits, unmoving scale. The internet’s diagnosis is usually “it stopped working.” The trial data says something more useful: this is what the medication working looks like, later in the curve.
First: is it actually a plateau?
Day-to-day weight swings by one or two kilograms on water, sodium, carbohydrate stores, hormones, and digestion timing. None of that is fat. Rules of thumb:
- A fluctuation is any story shorter than ~3 weeks, or any comparison between two single weigh-ins.
- A plateau is a flat weekly-average trend over 4+ weeks.
This is the strongest argument for weighing on a routine and looking only at the trend line (Glu plots exactly this): most “plateaus” people panic over are ten days of noise, and the panic produces exactly the diet-thrash that creates real problems. Check the trend before diagnosing anything.
Why real plateaus happen — even at full dose
Look at the published weight curves in STEP 1 (semaglutide) and SURMOUNT-1 (tirzepatide): rapid loss early, progressive flattening, then essentially stable average weight from roughly week 60 onward. Participants with perfect trial adherence plateaued. The average endpoint — about 15% loss on semaglutide 2.4 mg, about 21% on tirzepatide 15 mg — is the plateau, arrived at and held.
The mechanics: as weight falls, a smaller body needs fewer calories, and metabolic adaptation lowers energy expenditure a bit further than the size change alone predicts. Meanwhile the medication’s appetite effect is constant, not escalating. Intake and expenditure eventually re-meet, and weight stabilizes at the new equilibrium. The drug hasn’t quit; the easy arithmetic has been spent.
What your options actually are
1. Reframe — maybe nothing is wrong
If you’ve lost meaningful weight and the trend is flat, you may simply be at your result — the part of the curve every trial participant reached. The clinical question becomes maintenance: protecting the loss (see rebound weight gain), not forcing more. For a lot of people the honest move at the plateau is to stop treating it as a problem and start treating it as an arrival. Compare against your starting point, not against the loss rate of month two — and check what else moved: bloodwork, blood pressure, clothes, stamina, food noise. The scale is one instrument on the dashboard.
2. Check the dose ceiling
If you plateaued at a mid titration dose, there may be headroom — moving from 1 mg toward 2.4 mg semaglutide, or 5 mg toward higher tirzepatide steps, produced clearly larger average losses in the dose-response data. Whether the next step is worth its side effects is a prescriber conversation, and it’s a far more productive one when you arrive with a dose history and a weight trend instead of a feeling. A months-long flat trend at a sub-maximal dose is precisely the chart that changes prescriptions.
3. Audit the quiet drift
Appetite suppression commonly softens as months pass, and eating drifts upward without announcing itself — portions creep, calorie-dense convenience returns, the medication keeps the noise down while intake rises anyway. Two honest weeks of meal logging usually settles whether drift is part of your plateau. Same for movement: weight loss itself reduces daily calorie burn (you’re carrying less), so activity that outran your intake a year ago may only match it now.
4. Strength-train into the plateau
A plateau is the ideal moment to shift goals from losing to recomposing: resistance training plus adequate protein preserves or builds muscle at stable weight, which improves body composition and modestly supports metabolism — and it gives progress a scoreboard the scale can’t show (see muscle loss on GLP-1s).
What doesn’t work
Crash-restricting on top of the medication (accelerates lean mass loss and is unsustainable), skipping doses to “reset sensitivity” (no evidence, real regression risk — the gaps just lower your medication level; see missed doses), and scale-watching daily with rising anxiety (measurement noise, treated as news).
When a plateau plus symptoms means something else
A flat scale with returning hunger and food noise at full dose, or weight regain while on medication, is a different conversation than a quiet plateau — bring it to your prescriber. And an abrupt appetite return after a pharmacy switch is worth mentioning too; supply chains have produced real dose inconsistencies.
